SUMMARY OF PHYSICAL REQUIREMENTS
1. Able to speak, read, and write in English for task including assignments, taking/giving directions, and food/medication labels.
2. Able to communicate and respond clearly on telephone and respond to patients spoken needs.
3. Must possess the ability to physically transfer, lift, or assist patients whose average weight is 160 pounds with or without the aid of mechanical devices.

1. Follow each resident’s Plan of Care as directed by the care home operator including assistance with personal care including dressing/undressing, bath or shower, hair, skin, mouth, and nail care.
2. Monitor and record vital signs on a daily basis.
3. Assist residents with elimination using bathroom, commode, bedpan, or urinal.
4. Assist residents with ambulation, transfer activities, and prescribed exercise by instruction of the care home operator.
5. Assist with household services that are essential to the provision of assistance with ADLs.
6. Report pertinent observations of residents’ physical and psychological status to the care home operator.
7. Document accurately and concisely all care procedures, medication administration, and resident observations, and report any changes, concerns, or questions.
8. Adhere to Home’s policies and procedures. Participate in yearly skills evaluation and employee evaluation, and in pertinent in-service education and training programs.
9. Consistently report for duty on time and as scheduled.
10. Maintain strict confidentiality of home.
