VistaCare Hospice is a leading provider of hospice care services. Through
interdisciplinary teams of physicians, nurses, home healthcare aides, social
workers, spiritual and other counselors and volunteers, VistaCare provides

provided in the patient's home or other residence of choice, including

Patient Care Manager. Perform various services for a patient as necessary to
meet the patient's personal needs, to promote the patient's comfort, and a safe
environment. Observe patients and report/document these observations and the
services provided. Manage related expenditures in a fiscally responsible manner
in accordance with the Company's budget. Travel as necessary. Requirements• Minimum age requirement of 18.
• Completion of recognized Nurse Assistant certification program
• Passing score on Odyssey's skills test.
• Minimum of one-year direct patient care experience in an institutional or home
care setting required. Hospice experience preferred.
• Skilled in the proper technique for lifting and transferring patients.
• Ability to read, write and follow instructions and document care given.
• Understands Hospice philosophy and needs of terminally ill.
• Compliance with accepted professional standards and practices.
• Demonstrates tact, patience and good personal hygiene.
• Excellent observation and communication skills.
• Ability to work within an interdisciplinary setting.
• Physical exam in accordance with state law requirements.
• Valid driver's license with maintenance of a good driving record and reliable
transportation that is insured in accordance with state and/or Odyssey
requirements and is in good working order.
• Satisfactory references from employers and/or professional peers.
• Satisfactory criminal background check.
• Self-directed with the ability to work with little supervision.
• Flexible and cooperative in fulfilling all obligations.
