Family Choice is an IDPH (Illinois Department of Public Health) licensed homecare agency who provides adult caregivers (CNA and/or Personal Attendants/Caregivers) to assist anyone who is physically or cognitively impaired.
Aside from our private duty services servicing insurance and private pay clients, Family Choice has expanded its services to government agencies (eg. Department of Human Services, Veterans Affairs). As such the company is "constantly" looking for new employees with or without experience to join their roster of Personal Attendants & CNAs.
This is a part time job that can also lead to a full time 80 hours every two weeks.
Personal Attendants/Caregivers & Certified Nursing Assistants
- Resume with references
- All 2010-2011 Trainings received (including in-services attended)
- Current Drivers License (If you drive)
- Current Car Insurance (If you drive)
- Any one (1) of these: (SS Card, Current US Passport, Current Employment Authorization Card)
- Current 2011 Medical Reports
o TB Testing
o Hepatitis B (Proof of Shot)
o FLU Shot (If you have, but not mandatory)
o Physician's Exam
- Current CPR
- For CNA: CNA Certificate of successfully passing the Illinois Exam
