Job Summary:
Patriots Landing is looking for new TEAM members to ad to our growing Community!
CNA
Responsible for providing personal care assistance and daily activity support. Provide a safe and secure environment. Monitor and ensure the general well being of the residents. Assist with daily meal service. Assist with housekeeping and laundry services as needed.
Assist all residents with personal care needs, such as bathing, dressing, toileting, oral hygiene, daily reminders, transferring, ambulating, grooming; i.e., oral care, make-up, powder, deodorant, shaving, etc.
Communicate changes in the physical and/or emotional status of the resident and/or family. Report any medical issues to the Resident Care Coordinator, nurse on duty, or Assisted Living Director.
Follow assignment sheets for toileting needs, bath schedule, etc
Maintain a professional caring approach when dealing with residents, families, staff, visitors, vendors and the general public.
Qualifications:
Requires high school diploma or equivalent. Experience in working with the elderly and/or personal care preferred. Must possess the ability to read, write and speak English. Possess strong oral, written and telephone skills. Be dependable. Lift 50 pounds unassisted. The ability to work on your feet for eight hours a day, five days a week. Obtain and maintain First Aid/CPR certifications and be capable of meeting all requirements that may be specified by the state or other regulatory agencies. Obtain current Food Handlers permit.
LPN
Assist the Director of Assisted Living and Resident Care Coordinator with Quality Assurance, resident care plans, monthly wellness checks, assessments, staff training, medication ordering and medication check in. Provide day-to-day medication administration and back up support for shift routines. Monitor the general health and wellness of each resident. Practice proper sanitation and quality of care. Assist with hands-on personal care tasks.
Prepares and administers medications per policy.
Order and check in medication when delivered by the pharmacy.
Prepare residents' medications to accompany them on short-term absences from the community as allowed by state.
Document in residents' Progress Notes any change in status. Review Progress Notes regularly.
Complete resident treatments as ordered and scheduled.
Assist with Resident Assistant duties and team assignments as needed.
Relay resident health emergencies and/or status to Director of Assisted Living, Resident Care Coordinator, Executive Director/General Manager or Manager on Duty.
Weigh residents as scheduled. Check vital signs as part of the Wellness Program or as needed. Record information legibly in Progress Notes.
Assist with personal care and physical support. This may include assistance after a fall, assistance in transferring, assistance with bathing, dressing, oral hygiene, toileting, ambulation, grooming, etc.
Qualifications:
Must have a current and valid Washington state LPN license. A minimum of one year experience in a retirement or senior health care environment with experience in medication administration and hands on personal care preferred. Must possess the ability to read, write and speak English. Must possess excellent oral, written and telephone communication skills. Must be dependable, capable of making mature judgments and possess strong listening skills. Lift 50 pounds unassisted. The ability to work on your feet for eight hours a day, five days a week. Obtain and maintain First Aid/CPR certifications and be capable of meeting all requirements that may be specified by the state or other regulatory agencies. Location: DuPont
