Must be able to provide all medical requirements as set forth by the RI Dept of Health for health care workers.

• Proof of immunity to MMR ( by vaccine or by titer result )
• Proof of immunity to Varicella (by vaccine, titer result or history of disease as documented by physician)
• Proof of Tdap
• Proof of (2) negative PPD, or if positive PPD, Proof of negative CXR
• Physical within 1 yr (documented on our paperwork supplied at interview)
• Current BCI from Attorney General’s office dated within the last 30 days
• Must present social security card, drivers license, CPR Card and Insurance card at interview.
