RESPONSIBILITIES:
· Responsible for identifying patient/family needs and for providing supportive care and appropriate services for terminally ill patients in accordance with the Hospice plan of care.
QUALIFICATIONS:
· Current state license to practice as a Licensed Practical Nurse (LPN), CNA, and HHA's
· Minimum one (1) year of nursing experience required
· Current CPR certification
· Copy of updated Physical, based on state requirements
· Copy of updated TB results/ Chest x-ray
· 4hrs Alzheimer CEU
· Hippa CEU
